Forms that need to be signed (Release and Waiver, Scientific Diver Medical Eval., Temporary Diver Medical Eval.) should be MAILED to the Diving Safety Office at the address found in the following link:
All other forms may be faxed to (407) 646-2429 or e-mailed to the Diving Safety Officer.
